Occupational Therapist – Recovery Hubs (A Council in Leeds)\n \nContract: Locum | Full-time\n \nA council in Leeds is seeking an experienced Occupational Therapist to join the Recovery Hubs service, working as part of a multidisciplinary team supporting adults to regain independence and return safely to their own homes.\n\n The Recovery Hubs consist of Occupational Therapists working alongside Physiotherapists, Nurses, and support staff to deliver a rehabilitation-focused, 24-hour therapeutic approach aimed at promoting independence and positive outcomes for service users.\n \nMain Duties:\n\nComplete holistic Occupational Therapy assessments for adults with a range of needs within a reablement/rehabilitation setting\nDevelop and implement goal-led, strength-based outcome plans to support independence and recovery\nWork closely with support workers and wider MDT colleagues to ensure coordinated delivery of interventions and progress reviews\nLead on discharge planning for service users returning to their own homes\nApply moving and handling expertise, including risk assessment and identification of safe, person-centred solutions\nPromote single-handed care principles and reduced dependency where appropriate\nContribute to a 24-hour therapeutic, rehabilitation-focused model of care within the Recovery Hubs\nEnsure all interventions are delivered in line with council policies, statutory requirements, and professional standards\nRequirements:\n\nHCPC Registered Occupational Therapist\nExperience within community, reablement, or rehabilitation settings\nStrong assessment skills with a strengths-based, person-centred approach\nConfident in moving and handling risk assessment and intervention planning\nExperience working within multidisciplinary teams\nAbility to manage complex caseloads and support timely discharge planning\nWhy Join Our Agency?\n With over 10 years’ experience in social care recruitment, Ackerman Pierce provides a dedicated and supportive service throughout your placement.
